Your First Visit

Before your visit
In order to facilitate your first visit, you will need to fill out several forms and questionnaires, which are available on this Web site.  If you do not have access to a printer, we are happy to send the forms by mail. Let the office know that you need the forms mailed at the time of booking, or call (888) LUHS-888 and ask for extension 62180.

We ask that you arrive 15 minutes before your scheduled appointment. When you check in, the service representative will request the following:

  • insurance card and co-payment, if applicable
  • physician referral, if required by your insurance provider
  • copies of other medical records, if appropriate
  • current medication bottle(s), including herbal remedies
  • completed forms

Before your visit, it is also a good idea to jot down symptoms you have noticed as well as any questions you know you want to ask. Please bring this list with you.
The center is located on the third floor of the Loyola Outpatient Center, which is the furthest building south on the campus (click here for a campus map). Valet parking is available in the front of the building and there is a self-parking lot right next to the building.
