What to Bring to Your Appointment
Becoming a Patient

Please bring the following items with you to your appointment:

1. All medications, doses and directions, including non-prescription medications.
2. Copies of your films, if you’ve had any imaging studies, along with the physician’s interpretation.
3. Your Loyola patient ID card, if you have one.
4. Your insurance card or other coverage information.
5. A physician referral, if needed.
6. The name and address of the physician who referred you for this appointment.

In the event that you are unable to keep you appointment, please contact us 24 hours prior to your appointment so that we can offer your time to another patient.

To speak with a coordinator who can help assess your medical needs and find an appropriate physician, call the Loyola Physician Referral staff at (708) 327-1000.
