How to Refer to Heart Failure
As a leading teaching hospital with access to the latest in research and clinical trials, Loyola's cardiomyopathy and heart failure patients often benefit from life-saving techniques and procedures that may not be available elsewhere.

If you would like to discuss a case with any of our heart failure specialists, please call (877) MDs-LUHS.

Who should you refer with heart failure/heart transplant?

  • Heart failure patients with left ventricular systolic or diastolic dysfunction
  • Patients with signs and symptoms for heart failure despite appropriate medical regimen

Treatments available

  • Advance therapy for left ventricular dysfunction
  • Bi-ventricular pacing
  • Clearance for high risk revascularization
  • Heart transplant evaluation
  • Heart failure research protocols
  • Hemodynamic studies and diagnostic endomyocardial biopsy

Information requested for patients referred with heart failure

  • Electrocardiogram
  • Echocardiogram
  • Laboratory tests (i.e. BMP, TSH, etc)
  • Anticoagulation records
  • Stress test
  • Holter monitor
  • Recent physician notes
  • Angiogram report/film
