What to Bring to Your Appointment

Please bring the following items with you to your appointment:

    1. All medications, doses and directions, including non-prescription medications.
    2. Your insurance card or other coverage information.
    3. A physician referral, if needed.
    4. The name and address of the physician who referred you for this appointment.
    5. Any films, such as CT or MRI images, that you have had done recently. While the readings or written reports are useful, our physicians prefer to have the actual scans themselves.

In the event that you are unable to keep your appointment, please contact us 24 hours prior to your appointment so that we may offer your time to another patient.
