Once the patient has received treatment at Loyola University Health System (Loyola), we ensure a smooth transition back to the care of the referring physician. Physicians may refer patients by calling the liver transplant office at (708) 216-0364. An appointment will be set up for the patient to be seen in the clinic for evaluation of the liver disease and appropriateness for a liver transplant. At this time, the patient can be referred to a liver transplant coordinator with have immediate questions or concerns.

The patient should bring any medical records to the clinic on the day of the first appointment.
