Long-Term Discharge
Kidney Transplantation

Discharge plans begin soon after transplantation. While the recipient is still hospitalized, arrangements are made to provide the medications that will be taken after the transplant. As post-transplant blood tests are an important way to monitor the kidney function and medication levels, specially trained home-health nurses are often utilized to draw blood and monitor progress between clinic visits during the recovery process. The patient will have frequent, close contact with the transplant coordinators and physicians following the transplant. After approximately six months, the recipient is referred back to the referring physicians for their overall care, but the Loyola University Health System (Loyola) transplant team will continue to monitor blood work and kidney function. The team is always available for the transplant patients.
