Patient
Rights and Responsibilities
Loyola University
Health System (Loyola) is committed to excellence in
patient care. Your rights as a patient are everyone's
concern. We seek to provide you care that goes beyond
the treatment of your disease or disorder. We also treat
the human spirit.
Your well-being
is a collaborative effort among you and your Loyola
health-care providers. To that end, we share with you
both your rights and responsibilities as a patient.
We uphold these rights. If you, as a patient, do not
have your own decision-making authority, Loyola recognizes
your patient representative as having these rights
on your behalf, as allowed by law. Please see the attached
document on patient rights and responsibilities.
We
staff Loyola University Hospital with the number of
nurses needed to match patients' acuity, or severity
of illness, on a daily basis. Anyone can inquire about
our nursing staffing levels by calling Patient Relations
at (888) LUHS-888 and ask for extension 6-5140. We will
be happy to answer questions in accordance with the
2004 Illinois Hospital Report Card Act.
A patient
bill of rights is located in each patient's room and
in the main entrance of all Loyola University Health
System facilities.
