Questions
and Answers About Loyola Insurance Plans
How do I know if the
health insurance plan I am considering uses Loyola University
Health System (Loyola)?
Talk to a
representative of the health insurance plan. Ask if
the plan contracts with Loyola University Medical Center/Loyola
University Physician Foundation-affiliated physicians,
hospitals and other
services, and ask how to maximize your benefits. If
your health insurance plan requires you to use a primary
care physician, ask that physician if he or she will
refer you to an LUHS-affiliated provider when necessary
and appropriate.
The top organizations that LUHS contracts with are as
follows:
Blue Cross/Blue Shield of Illinois (includes HMO
Illinois)
Cigna
Great West Healthcare
HealthCare's Finest Network (HFN)
Humana
Preferred Plan
Private HealthCare Systems (PHCS)
United Healthcare
Unicare
LUHS physicians also participate as primary care physicians
in the following HMO/POS organizations:
Blue Choice POS
Great West Healthcare HMO/PPO/POS (excludes OnePlan/Open
Access)
HMO Illinois (Site 349)
UHC HMO/POS/EPO

care to hospitalization, rehabilitation and home care.
