methods
Electronic Funds Transfer (EFT)
EFT gives you the greatest flexibility as a member of LCHF. By selecting the EFT option for your monthly dues payment, you are never locked into your membership for any time period and you may cancel at anytime.
When you elect for EFT, you will automatically be billed to your account of choice. However, you may only have one account billed each month for the entire membership. Membership dues may not be divided and billed on two separate EFT accounts. The permitted billing sources include the following:
- Mastercard, Visa or Discover
- Checking account
- Savings account
In addition, an EFT account will make you eligible for house account privileges with written authorization.
In the event that the EFT from any billing source by denied, a $5 monthly service fee will be assessed if the outstanding balance is not collected by the 25th of the existing month. The monthly service fee for returned billing is non-refundable and non-negotiable.
6- or 12-Month Pre-Payment
Members may elect to pay for 6 or 12 months of membership in advance. Any increase in monthly dues will not be reflected until the end of the 6- or 12-month time period. Dues are non-refundable when paid in full for 6 or 12 months. At the end of the 6-or 12-month time period, each member will be notified of his/her upcoming expiration date. To continue membership after expiration, members must provide an additional 6- or 12-month pre-payment or provide an account for EFT.
Loyola University Health System Payroll Deduction
Loyola University Health System employees are required to have monthly dues payment through payroll deduction. Monthly dues are deducted from the employee's paycheck on a monthly or bi-monthly basis, depending on individual payroll frequency. All payroll deductions will continue to be deducted from the member's paycheck until the membership is canceled in writing or the employee is terminated from the Loyola University Health System.
