uidelines for Completion of Quality Monitor
Report
If you are reporting a Quality Monitor, complete all of the
items on the quality monitor report form. Graphical display of data should be
inserted into the body of the report or sent along with the report. It is
recommended that reports be submitted in electronic form (via email, if possible) to
William Barron, M.D. or LuAnn Vis, Center for Clinical Effectiveness.
Indicator Category
Click on the link to view the categories and choose the one that corresponds best
with the focus of your quality monitor.
Target Rate
The target rate (desired goal) is the specific measurable goal for performance.
Source of Target Rate
This may be an external benchmark or an internally set goal. If external,
please cite the source of the target rate.
Results - Attach data in graph form
- Present quantitative results of the outcome studied. It is strongly
suggested that results be presented in graphical format to highlight changes over time
(contact the CCE at 6-3290 if assistance with graphical presentation and/or statistical
analysis is desired). Please avoid inclusion of spreadsheets containing large
amounts of data – such information is often very difficult to interpret.
Analysis of Data and Next Steps
- Provide a narrative description of the findings to accompany the
graphic presented above.
