  • Can I get a medical consultation online?

    For legal purposes, our physicians cannot respond to questions regarding diagnoses or treatments online. It would be considered unprofessional conduct for a medical professional to provide treatment, electronically or otherwise, without an adequate patient history or physical examination to establish a diagnosis, identify underlying conditions and determine contraindications to treatments or prescriptions. We recommend you make an appointment to see an LUHS physician by calling (708) 216-8563. If you need assistance in selecting a physician, please call the LUHS Physician Referral staff at (708) 327-1000 to speak with an associate who may better assist you.

  • If I provide you my diagnosis, can you tell me which physician I need to see?

    Currently we do not have a mechanism in place to provide online physician referral. To speak with an associate who may better assist you, contact the LUHS Physician Referral staff at (708) 327-1000.

  • How do I get the name, e-mail address, phone number, or mailing address for a physician at Loyola?

    Many of our physicians do not commmunicate with patients via e-mail. We encourage you to call your physician's office or nurse directly with your question. Please see our Programs & Services page for contact information. If you are a patient, please call (888) LUHS-888. If you are a referring physician, please call (877) MDs-LUHS.

  • How can I get a copy of my (or a loved one's) LUHS medical record?

    You need to fill out an "Authorization for Disclosure of Medical Record Information" form and return it with the appropriate fees to:

    You can either download the form from this web site or you can call (708) 216-3665 to request a form. If the records are for a deceased patient, you must provide legal documentation showing power of attorney, executor of the estate, or a death certificate showing next-of-kin to obtain records.

    If you are not requesting specific documentation, you will be sent an abstract that typically includes history and physical reports, discharge summary, operative reports, consultations, lab, X-ray, specialized tests and most recent outpatient visits.

    Charges
    For ease of reference, hospitals may be reimbursed for copies of medical records as follows: 

    • $0.86 per page for pages 1-25, $0.57 per page for pages 26-50 and $0.29 per page for pages 51 and up.
    • $1.43 per page for copies made from microfiche or microfilm.
    • Actual postage will be charged for all copies.
    Records being sent to a physician or a health-care facility are provided at no charge to the patient.

    VALID ID MUST BE SHOWN AT TIME RECORDS ARE PICKED UP.

    Requirements of this statutory section shall be satisfied within 60 days of the receipt of a request by a patient, or attorney.

    The statutory reference, for your information, is 735 ILCS 5/8-2001. The fees will be adjusted yearly commencing on 1/20/03.

    Time
    Most requests are processed within 10 business days of receipt.
